We assist with care coordination, helping you understand appointments and test results, conducting research and finding resources, and other support that makes your health care journey smoother.
Absolutely not. Care Concierge is designed to complement your healthcare, not replace your Primary Care Provider (PCP). We support you by helping you prepare for appointments, understand recommendations and navigate your healthcare journey more confidently. We encourage you to maintain a relationship with your doctor, and we’ll help make that relationship even more effective.
That’s okay! You can still use Care Concierge. We’ll support you with health system navigation, appointment preparation, and personalized guidance while you search for a PCP. If needed, we can also help you understand your options for finding a PCP in your area.
We will email you with registration information and an access code. Once you activate the access code, you can schedule your first appointment where your care coordinator will aim to understand your needs and start to build a trusted relationship.
If there is room in the category to which you want to switch, you can work with you care coordinator to do so.
You can join the waitlist, and if a spot becomes available, we’ll notify you.
If we notice inactivity for an extended period, we’ll reach out to see how you are. If you no longer need the service or we don’t hear back, your spot may be offered to someone on our waitlist.
