How the Care Concierge Public Access
Program Works

1. Sign Up to Secure Your Spot

Enter your information in the form above to reserve a place in the Program. Upon successful registration, you'll receive instructions for downloading the Serefin Health app and a unique access code.

2. Schedule & Complete an Intake Session

Once you have successfully logged into the Serefin Health app and set up your profile, you can schedule an intake session. You will be matched with a care coordinator who will contact you at the scheduled time.

3. Enjoy Ongoing Complimentary Support

Once you've completed your intake session, enjoy custom care coordination, expert guidance and access to tailored resources for as long as you need. Our secure platform allows individuals to connect with the Care Concierge team via video and phone calls, messaging and a user-friendly dashboard—giving you timely access to personalized support tools wherever you are.
